Roles & Responsibilities

Provide compassionate, patient-focused nursing care to assigned residents
Administer medications and treatments safely and accurately
Perform thorough assessments and document findings in the medical record
Collaborate with nurses, physicians, and care team members to implement care plans
Monitor patient conditions and escalate concerns promptly to ensure safety
Support residents with activities of daily living while promoting dignity and independence
Participate in facility quality improvement and infection control practices

Qualifications

Active LPN license (Pennsylvania licensure or ability to obtain state licensure)
Graduate of an accredited practical nursing program
Current BLS/CPR certification or willingness to obtain upon hire
Strong clinical assessment and medication administration skills
Excellent communication and teamwork abilities
Reliable, compassionate, and committed to high-quality patient care

Education

Graduate of an accredited Practical Nursing (LPN) program
High school diploma or GED

Knowledge & Skills

Proficient in basic nursing assessments and documentation
Medication administration and IV/PO route familiarity
Knowledge of infection control and patient safety practices
Comfortable working in a collaborative, fast-paced healthcare setting
Good time management and prioritization skills
Ability to use electronic medical records and bedside charting (as available)
