Contract Type: Per-Diem

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care for residents and patients
Administer medications and treatments safely and accurately
Monitor and document vital signs, patient status, and care interventions
Collaborate closely with RNs, physicians, and the care team to ensure quality outcomes
Perform wound care, IV assistance, and other clinical tasks within LPN scope
Educate patients and families about care plans and discharge instructions
Maintain accurate, timely charting in the electronic medical record
Adhere to infection control, safety standards, and facility policies

Qualifications

Active LPN license in Pennsylvania required
Completion of an accredited LPN/LVN program
Current BLS/CPR certification required or obtained upon hire
Previous clinical experience in long-term care, rehab, or acute settings preferred
Strong communication, organizational, and teamwork skills
Up-to-date immunizations and ability to pass required background checks

Education

Graduation from an accredited LPN/LVN program
High school diploma or GED

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and dressing change techniques
Proficient in taking and interpreting vital signs
Familiarity with electronic medical records and charting
Understanding of infection control and patient safety principles
Ability to prioritize tasks and respond to changing clinical needs
