Roles & Responsibilities

Provide nursing care for clinic patients following established standards and practices, working within the scope of LPN licensure.
Serve as the primary liaison between patients, clinical staff, and providers to ensure smooth, patient-centered care.
Take vital signs and perform focused patient assessments to support provider decision-making.
Triage patient calls, prioritize care needs, and coordinate appropriate follow-up.
Manage medication refill requests and complete necessary prior authorizations in a timely manner.
Give direction to CNAs and schedulers while receiving guidance from providers, coordinators, clinical managers/supervisors, and RNs.
Collaborate with the interdisciplinary team to meet patient needs and maintain clinic workflow.
Maintain required continuing education and comply with state and federal licensure requirements.

Qualifications

Graduate of an accredited nursing program (LPN/LVN program) as stated in the job description.
Current State LPN license (must be active and in good standing).
BLS (Basic Life Support) certification—documentation required.
Demonstrated ability to take vital signs, triage patients, manage medication refills, and complete prior authorizations.
Comfort providing direction to CNAs and working closely with schedulers and clinical staff.
Willingness to work weekdays with occasional weekend coverage.
