Roles & Responsibilities
- Provide direct nursing care to clinic patients within the scope of LPN licensure, ensuring safety and high-quality care
- Act as the primary liaison between patients, clinic staff, and providers to coordinate care and follow-up
- Perform patient assessments including taking vital signs and documenting findings accurately
- Triage patient calls and determine appropriate next steps or escalation to providers
- Process prescription refill requests and complete necessary prior authorizations promptly
- Support the care team by delegating tasks to CNAs and Schedulers as appropriate
- Collaborate with interdisciplinary team members to meet patient needs and clinic goals
Qualifications
- Current State LPN license in good standing
- BLS (Basic Life Support) certification
- Graduate of an accredited Nursing Program
- Ability to perform patient assessments, triage calls, and manage prescription refill/prior authorization workflows
- Strong communication and teamwork skills; comfortable giving direction to CNAs and Schedulers and receiving direction from RNs and providers
Education
- Graduate of a Nursing Program
Knowledge & Skills
- Patient assessment and vital signs monitoring
- Triage of patient calls and clinical prioritization
- Prescription refill processing and prior authorization workflows
- Delegation and team coordination with CNAs and Schedulers
- Interdisciplinary collaboration and clear clinical communication
- Basic Life Support procedures
