Roles & Responsibilities

• Administer medications and treatments accurately and safely to residents
• Perform routine assessments and monitor resident vitals, reporting changes promptly
• Provide hands-on care and assist with activities of daily living with compassion
• Document care in the electronic medical record promptly and thoroughly
• Collaborate with nursing staff, therapists, and providers to implement individualized care plans
• Participate in wound care, dressing changes, and other skilled nursing tasks as needed
• Support infection control practices and maintain a safe environment for residents
• Communicate effectively with residents and families to educate and address concerns

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico (required)
• Current BLS/CPR certification (required)
• Experience in long-term care or skilled nursing preferred
• Valid immunizations and ability to pass background and drug screening
• Strong communication skills and a team-oriented approach
• Flexibility to work a consistent 6:00 AM - 6:30 PM schedule on assigned days

Education

• Graduation from an accredited Practical Nursing program; LPN diploma or certificate

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment and basic wound care knowledge
• Familiarity with electronic medical records and charting
• Infection control and safety procedures
• Time management for long shifts and high-acuity tasks
• Compassionate patient-centered care and effective communication
