Roles & Responsibilities

Provide comprehensive outpatient family medicine care for patients across the lifespan
Build and maintain long-term, patient-centered relationships with individuals and families
Deliver continuity of care with thoughtful follow-up and care coordination
Collaborate closely with clinical and administrative staff to ensure high-quality, team-based care
Contribute to community-focused health initiatives that make a real impact locally

Qualifications

MD or DO degree from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Eligible for or holds medical licensure in New York State
DEA certification or eligibility
Passion for community-focused, patient-centered healthcare

Education

MD or DO degree from an accredited medical school

Knowledge & Skills

Primary care and outpatient-focused clinical expertise
Continuity of care and patient-centered medicine
Ability to care for patients across the lifespan
Experience working effectively within a collaborative care team
Strong communication and relationship-building skills with patients and staff
