Roles & Responsibilities

• Implement occupational therapy treatment plans for short-term and long-term residents to support functional independence
• Collaborate closely with the supervising Occupational Therapist and the interdisciplinary care team to coordinate patient-centered care
• Complete timely, accurate documentation in Net Health in accordance with facility standards
• Maintain compliance with facility policies, Medicare guidelines, and regulatory standards to ensure high-quality care
• Manage caseloads efficiently using strong time-management skills to maximize patient outcomes
• Offer mentorship and support to less experienced staff when appropriate, contributing to a culture of learning

Qualifications

• Active OTA License (required)
• Experience in a Skilled Nursing Facility (SNF) preferred — mentorship available for candidates with 0-1 year experience
• Comfortable and experienced with documentation workflows (Net Health experience is a plus)
• Strong time-management skills and ability to prioritize a dynamic caseload
• Flexible, collaborative mindset and strong interdisciplinary communication skills
