Roles & Responsibilities

• Assess resident condition using the nursing process and adjust care plans accordingly
• Implement physician-ordered treatments and administer medications safely
• Document assessments, interventions and outcomes accurately and promptly
• Interpret diagnostic tests to inform resident care and escalation when needed
• Collaborate with interdisciplinary team members to coordinate resident-focused care
• Provide resident and family education to support recovery, safety and quality of life
• Participate in performance improvement activities that enhance resident outcomes

Qualifications

• Current California RN license in good standing
• Current CPR certification (BLS) required
• Graduate of an accredited school of professional nursing
• At least 1 year of experience as a Registered Nurse (SNF experience preferred)
• Comfortable working night shift 12-hour rotations

Education

• Graduate of an accredited school of professional nursing

• Proficient with the nursing process: assessment, diagnosis, planning, implementation and evaluation
• Medication administration and safe dosing practices
• Interpreting diagnostic tests and communicating findings
• Care planning and resident-centered assessment skills
• Accurate clinical documentation and charting
• Resident education and effective patient/family communication
• Collaboration with interdisciplinary teams in a Skilled Nursing Facility (SNF) setting
