Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans
• Deliver hands-on and progressive therapeutic interventions to restore function and mobility
• Document clinical findings, progress notes and discharge summaries accurately in the EMR
• Collaborate with nursing, therapy aides and interdisciplinary teams to coordinate care
• Educate patients and families on home programs, safety and long-term recovery strategies
• Lead restorative programs, gait training, transfers and fall-prevention protocols
• Supervise and mentor therapy assistants and students when applicable
• Contribute to discharge planning and transitions of care with a patient-centered focus

Qualifications

• Active Physical Therapist license in Mississippi (required)
• Current BLS/CPR certification (required)
• Experience working in skilled nursing, rehabilitation, or long-term care settings preferred
• Comfort with floating between facilities and adapting to different care teams
• Strong clinical reasoning, documentation skills and patient-centered communication
• Professional, punctual, and able to manage a varied caseload independently

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy acceptable where licensed

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise and mobility training
• Knowledge of restorative nursing and long-term care rehabilitation best practices
• Proficient with common modalities, manual therapy techniques and assistive devices
• Familiarity with documentation standards and electronic medical records
• Excellent interpersonal and education skills for working with patients and families
• Ability to collaborate effectively with multidisciplinary teams
