Roles & Responsibilities

• Assess patients' physical function and mobility to create individualized rehab plans
• Plan and deliver evidence-based therapeutic interventions to restore function
• Measure progress and adjust treatment plans to maximize patient outcomes
• Document care thoroughly and accurately in the medical record
• Collaborate with nurses, physicians, and rehab team members for coordinated care
• Educate patients and families on home exercise programs and safety strategies
• Support safe discharges by contributing to functional and discharge planning

Qualifications

• Current, unrestricted Physical Therapist license in California
• BLS/CPR certification required
• Experience in outpatient, subacute, or skilled nursing rehab is a plus
• Comfortable working a Tuesday-Saturday schedule and adapting to facility processes
• Strong communication skills and a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy

Knowledge & Skills

• Skilled in gait training, therapeutic exercise, balance and fall prevention
• Proficient with assistive devices and adaptive equipment
• Familiarity with outcomes measurement and functional assessment tools
• Solid documentation skills and comfort using electronic medical records
• Ability to collaborate effectively within an interdisciplinary care team
