Roles & Responsibilities
- Provide compassionate, skilled occupational therapy services to skilled nursing facility residents
- Collaborate with interdisciplinary team members to create meaningful, patient-centered care plans
- Deliver therapeutic interventions to improve daily living skills, mobility, and independence
- Document progress, outcomes, and discharge planning in a timely and compliant manner
- Communicate clearly with residents, families, and staff to set realistic goals and celebrate gains
- Adapt treatment approaches when floating between Middletown and Centerville to meet resident needs
Qualifications
- Active COTA certification (Certified Occupational Therapy Assistant) or eligibility to obtain state certification
- Graduate of an accredited OTA program; associate degree typically required
- Comfortable working in a Skilled Nursing Facility (SNF) setting
- Open to clinicians at all experience levels including new grads
- Ability to float to a sister location in Centerville as needed
- Strong documentation skills and ability to work collaboratively on a therapy team
Education
- Graduate of an accredited Occupational Therapy Assistant (OTA) program (Associate degree preferred)
Knowledge & Skills
- Foundational occupational therapy principles and adaptive equipment knowledge
- Skilled in resident assessment, treatment planning, and progress documentation
- Familiarity with SNF workflows, regulatory documentation, and rehab-focused goals
- Good communication and interpersonal skills for resident and family interactions
- Ability to prioritize caseload and manage time efficiently during day shifts
