Roles & Responsibilities

• Evaluate residents and develop individualized treatment plans that promote independence in daily activities
• Provide skilled occupational therapy interventions in accordance with physician orders
• Target improvements in ADLs, mobility, strength, coordination, and cognitive skills
• Document evaluations, progress notes, and discharge summaries in compliance with facility and regulatory guidelines
• Collaborate with an interdisciplinary team including PT, SLP, nursing, and physicians to ensure coordinated care
• Educate patients and families on treatment plans and home exercise programs to support recovery
• Ensure compliance with state, federal, and facility regulations while delivering high-quality care

Qualifications

• Active California Occupational Therapist license (required)
• Graduate of an accredited Occupational Therapy program
• Current CPR/BLS certification
• SNF experience preferred
• Strong clinical reasoning and documentation skills
• Ability to work independently and as part of a multidisciplinary team

Education

• Graduate of an accredited Occupational Therapy program

Knowledge & Skills

• Skilled in occupational therapy evaluation and treatment planning
• Clinical reasoning and evidence-based decision making
• Strong documentation and charting abilities
• Effective communication and teamwork with interdisciplinary staff
• Knowledge of ADLs, mobility training, and cognitive interventions
• Familiarity with SNF regulatory and compliance requirements
