Shift Type: Monday-Friday, 7:00 AM - 3:00 PM (Day) — 8-hour days; start times are flexible as long as consistent and communicated with DOR (typical start between 6:00 AM and 10:00 AM). 35 GWW

Roles & Responsibilities
- Evaluate and screen patients with communication, cognitive, or swallowing disorders and develop individualized, evidence-based plans of care
- Utilize standardized assessments and clinical best practices to guide interventions and measure progress
- Assess patient progress regularly and adjust treatment plans to meet changing needs and regulatory standards
- Manage dysphagia and swallowing care safely and effectively using current protocols
- Document all regulatory and clinically required information accurately in the electronic health record
- Record treatment minutes and produce comprehensive discharge summaries that reflect outcomes and care provided
- Provide counseling and education to patients, families, caregivers, and other providers to support continuity of care
- Communicate clearly with supervisors and interdisciplinary team members about progress, barriers, and recommendations
- Participate in patient care conferences, utilization reviews, and facility trainings to support strong team-based care
- Obtain necessary DME or supplies per clinical and regulatory requirements and support smooth patient transitions
Qualifications
- Active Missouri State License (required)
- ASHA Certificate of Clinical Competence (CCC) (required)
- Minimum 1 year of SLP experience (required)
- Experience in LTC or SNF settings (required)
- Ability to supervise and support CFY and Student SLPs
- Willingness to provide required documentation at submission (state license, CCC)
- One reference accepted at time of submission
- 3 years active CCS preferred
Knowledge & Skills
- Skilled in speech-language pathology assessment and rehabilitation
- Experience with standardized assessments and evidence-based clinical interventions
- Dysphagia management and safe swallow strategies
- Strong clinical documentation skills in electronic health record systems
- Accurate recording of treatment minutes and outcome evaluations
- Comfortable consulting and counseling patients, families, and caregivers
- Effective interdisciplinary team communication and participation in care planning
- Familiarity with DME/supplies procurement and facility workflows
