Roles & Responsibilities

• Assess patients’ mobility, strength, balance, and functional limitations to create individualized care plans
• Develop and implement evidence-based therapeutic interventions to improve patient outcomes
• Deliver skilled inpatient and outpatient therapy focusing on safe discharge planning and return-to-function
• Document treatment progress and outcomes clearly and timely in the electronic medical record
• Collaborate with physicians, nurses, and the interdisciplinary team to coordinate patient-centered care
• Educate patients and caregivers on home exercise programs and safe activity progression
• Adapt treatment plans based on ongoing re-assessment and evolving patient goals
• Participate in quality improvement initiatives and clinical competency evaluations

Qualifications

• Active Physical Therapy license (state licensure verification required prior to start)
• Basic Life Support (BLS) certification
• Completion of Prime Core Competencies Exam, Prime Skills Checklist, and Prime Specialty Exam as required
• Current immunizations and health screenings as listed in job requirements (COVID-19 vaccination, TB testing, MMR, Hepatitis B, Varicella, Tdap, influenza, drug screening, fit test)
• Comfortable working in both inpatient and outpatient settings
• Strong documentation skills and familiarity with clinical outcome measures
• Excellent communication skills and a collaborative approach to care

Education

• Graduation from an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, gait training, and balance interventions
• Knowledge of discharge planning, functional outcome assessment, and safe mobility strategies
• Ability to prioritize caseloads and manage time efficiently in a fast-paced setting
• Proficient in patient and family education techniques and motivational coaching
• Familiarity with electronic medical records and clinical documentation best practices
• Team-oriented mindset with strong problem-solving and critical thinking skills
