Roles & Responsibilities

• Assess patients' mobility, strength, balance and functional needs to create meaningful treatment plans
• Develop and implement individualized therapeutic interventions that help patients regain independence
• Document patient progress clearly and timely in Cerner to ensure continuity of care
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Educate patients and families on home exercise programs and safe mobility strategies
• Participate in unit-based performance improvement activities and competency assessments (Trinity testing/checklist)
• Maintain professional licensure and complete required health screenings and immunizations

Qualifications

• Active Physical Therapist license in Iowa or eligibility to obtain one
• Basic Life Support (BLS) certification required
• Experience with Cerner or similar electronic medical record systems preferred
• Willingness to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to pass required health screenings (drug screen, TB test or IGRA, immunizations) prior to start

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, gait training and fall prevention
• Strong clinical reasoning and documentation skills in an acute care setting
• Comfortable collaborating with physicians, nurses and rehab team members
• Familiarity with Cerner for charting and order management
• Good communication and patient/family education abilities
