Roles & Responsibilities

• Conduct comprehensive SLP evaluations for skilled nursing residents to determine needs and goals
• Provide daily, patient-centered treatment sessions and implement individualized care plans
• Manage timely and effective discharges, ensuring smooth transitions of care
• Document progress, treatment plans, and outcomes accurately and promptly
• Collaborate with nursing and interdisciplinary therapy teams to coordinate care
• Meet facility productivity expectations (90% productivity requirement) while maintaining high-quality care
• Adhere to Continuum Rehabilitation Group (CRG) policies and facility procedures
• Maintain professional presence onsite during scheduled hours to receive guaranteed hours

Qualifications

• Active SLP license or certification appropriate for practice in New Mexico (state license required)
• ASHA CCC-SLP preferred where applicable
• 1-3 years of clinical SLP experience (as stated in the job posting)
• Prior SNF experience strongly preferred
• Comfortable working in a skilled nursing facility setting and collaborating within a therapy team
• Willingness to follow facility dress code (black scrubs) and CRG policies
• Available to begin on or around 2025-12-01 (requested start date)

Knowledge & Skills

• Skilled in evaluations, treatment planning, and discharge planning for SNF populations
• Strong documentation and clinical note-writing abilities
• Ability to work productively and meet productivity targets
• Good interdisciplinary communication and collaboration skills
• Experience with resident-centered goals and geriatrics-focused SLP care
• Familiarity with SNF workflows, regulations, and daily treatment rhythms
