Roles & Responsibilities

• Perform comprehensive speech-language evaluations for SNF residents
• Deliver consistent, high-quality daily treatments tailored to each patient
• Plan and execute safe and effective discharge recommendations
• Document patient progress thoroughly and maintain productivity expectations
• Collaborate with the interdisciplinary therapy team to optimize patient outcomes
• Adhere to CRG policies and procedures and facility-specific protocols
• Maintain professional presence on-site to receive guaranteed hours and support continuity of care

Qualifications

• Primary skill: SLP (Speech-Language Pathologist)
• 1-3 years of relevant clinical experience (as listed in the posting)
• Prior SNF experience strongly preferred
• Current state SLP license and/or ASHA certification encouraged (confirm facility requirements prior to start)
• Ability to meet productivity expectations (90% productivity requirement noted)
• Comfort working on-site in required dress code (black scrubs) and following facility policies

Knowledge & Skills

• Skilled in conducting speech and swallowing evaluations
• Experience delivering individualized daily treatment plans
• Proficient with documentation and discharge planning in a SNF setting
• Strong teamwork and communication skills within interdisciplinary rehab teams
• Familiarity with SNF workflow and productivity standards
• Professionalism in on-site presence and patient interactions
