Roles & Responsibilities

• Provide skilled physical therapy evaluations and individualized treatment plans for SNF residents
• Deliver hands-on and therapeutic interventions that improve mobility, function, and quality of life
• Collaborate closely with nurses, therapists, and facility staff to coordinate care and discharge planning
• Document clinical progress clearly and timely, supporting productivity and compliance goals
• Meet facility productivity expectations (90%) while maintaining high-quality patient care
• Adhere to CRG policies and procedures and represent the therapy team professionally (black scrubs required)
• Be present in the building to receive guaranteed hours and maintain continuity of care

Qualifications

• PT or DPT credential (DPT is listed as a primary skill)
• 1-3 years of experience working in a skilled nursing facility or similar rehab setting (per posting)
• Comfortable working on-site and following facility dress code (black scrubs)
• Ability to meet productivity requirements and follow CRG policies & procedures
• Strong communication and teamwork skills; willingness to collaborate with a multidisciplinary team

Education

• DPT preferred (DPT listed in job skills) or equivalent PT credential

Knowledge & Skills

• Experience with SNF-specific evaluation and treatment approaches
• Skilled in mobility training, therapeutic exercise, gait training, and functional transfer training
• Ability to create practical, resident-centered plans of care
• Efficient documentation practices and awareness of productivity metrics
• Professionalism, time management, and strong patient rapport-building
