Location: Woodland, California

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with occasional Saturday rotations once per month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs and develop individualized care plans
Instruct patients and families in therapeutic exercises and use of equipment
Report patient progress and changes to physicians
Supervise home health aides and participate in case conferences
Prepare clinical summaries and maintain accurate documentation
Ensure compliance with Medicare, Medi-Cal, OSHA, and other regulations

Qualifications

Minimum 2 years experience as a Physical Therapist in an acute, home care, or rehab setting
Valid Physical Therapy license in California
BLS certification required
Excellent oral and written communication skills
Strong organizational skills
