Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM; rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide home health physical therapy services including evaluations, treatments, and patient education
Conduct start of care, recertifications, and discharge visits
Collaborate with clinical manager and educator for support
Manage a caseload of patients with common diagnoses such as CHF, ortho, and pneumonia
Document patient progress and maintain compliance with regulatory standards
Utilize provided phone and tablet for efficient documentation and communication

Qualifications

Active Physical Therapist license in Kentucky or compact eligible
BLS certification required
Home health experience preferred, but new grads considered
Ability to work independently and travel to patient homes
Strong clinical reasoning and time management skills

Education

Graduate of an accredited physical therapy program (DPT or MSPT)

Knowledge & Skills

Expertise in home health therapy protocols and regulations
Proficiency in OASIS documentation
Excellent interpersonal and communication skills
Ability to adapt to various patient environments
Compassionate patient-centered care approach
