Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide physical therapy services to home health and hospice patients within a 60-mile radius of Valley City
Develop and implement individualized treatment plans for patients recovering from surgeries, falls, and other conditions
Conduct assessments and document patient progress using HomeCare/HomeBase EMR
Collaborate with interdisciplinary team to ensure comprehensive patient care
Travel to patient homes and manage a caseload of 5-7 patients per day

Qualifications

Must hold an active ND Physical Therapy License
Current BLS certification required
Valid Driver's License and Auto Insurance are a must
Minimum 1 year of home health experience preferred
Ability to travel within a 60-mile radius

Knowledge & Skills

Experience with HomeCare/HomeBase EMR is a plus
Strong clinical skills in physical therapy for diverse patient populations
Excellent communication and time management skills
Ability to work independently in a home health setting
