Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive Oasis Assessments and Start of Care evaluations for patients across four counties
Develop individualized physical therapy treatment plans focusing on falls prevention and fracture rehabilitation
Provide skilled therapy services in primarily rural settings, with visits averaging 4-6 patients per day
Collaborate with interdisciplinary team to ensure coordinated care and optimal patient outcomes
Maintain accurate clinical documentation using provided tablet technology

Qualifications

Current Physical Therapy license in the state of Indiana
Experience with Oasis Assessments and Start of Care documentation preferred
Ability to travel within Noble, Lagrange, Steuben, and DeKalb counties
Comfortable working in rural and small-town environments

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Excellent communication and patient education abilities
Proficiency in electronic health records and point-of-care documentation
Knowledge of home health regulations and Medicare guidelines
