Roles & Responsibilities

• Conduct comprehensive patient evaluations to create individualized care plans
• Deliver daily physical therapy treatments focused on returning patients home safely
• Plan and coordinate timely discharges in collaboration with the interdisciplinary team
• Document patient progress thoroughly and maintain accurate clinical records
• Collaborate with therapists, nursing, and support staff to optimize patient outcomes
• Maintain productivity standards and follow CRG policies and procedures

Qualifications

• Licensed Physical Therapist or eligibility for licensure in New Mexico
• DPT or equivalent PT degree (DPT preferred)
• 1-3 years of clinical experience; prior SNF experience strongly preferred
• Comfortable working in a skilled nursing facility environment with a high rehab-to-home population
• Professional, dependable, and able to be on-site to receive guaranteed hours
• Willingness to follow facility dress code (black scrubs) and CRG policies

Education

• DPT preferred; PT degree required or equivalent

Knowledge & Skills

• Proficiency in patient evaluations, treatment planning, and hands-on therapy
• Experience with discharge planning and transitions of care
• Strong clinical documentation skills and time management
• Ability to work effectively within an interdisciplinary rehab team
• Good communication skills and a patient-centered approach
