Roles & Responsibilities

• Provide compassionate, patient-centered nursing care that makes a real difference
• Administer medications and treatments accurately and document appropriately
• Monitor and record vital signs, reporting changes to the care team promptly
• Assist with ADLs and coordinate care with nurses, aides, and providers
• Perform wound care and other clinical procedures within scope of practice
• Maintain clear, timely charting in the electronic medical record
• Communicate effectively with patients and families to support comfort and understanding

Qualifications

• Valid Pennsylvania LPN license in good standing required
• Current BLS/CPR certification required
• Completion of an accredited practical nursing program
• Ability to pass background check and drug screening as required
• Strong communication skills and a collaborative attitude
• Prior experience in long term care, med-surg, or similar settings is a plus

Education

• Graduate of an accredited LPN/LVN program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic patient assessment and clinical monitoring
• Wound care basics and familiar with common nursing procedures
• Comfortable using electronic medical records and point-of-care charting
• Good time management and ability to prioritize patient needs
• Team-oriented approach and clear communication with interdisciplinary staff
