Roles & Responsibilities
- Provide compassionate, direct patient care that supports comfort and recovery
- Administer medications and treatments according to care plans and policies
- Monitor and document patient status, reporting changes promptly to the care team
- Assist with activities of daily living while promoting patient dignity and independence
- Collaborate with nurses, therapists, and support staff to coordinate excellent care
- Participate in safety and quality initiatives to improve patient outcomes
Qualifications
- Current LPN license in good standing (Pennsylvania license preferred)
- Completion of an accredited LPN/Vocational Nursing program
- Basic Life Support (BLS) certification or willingness to obtain upon hire
- Current immunizations and ability to pass required health screenings
- Positive background check and drug screening as required by facility
Education
- Graduate of an accredited LPN/Vocational Nursing program; high school diploma or equivalent
Knowledge & Skills
- Strong patient assessment and clinical observation skills
- Clear, compassionate communication with patients, families, and colleagues
- Comfort with charting and accurate documentation practices
- Ability to follow care plans and facility policies consistently
- Teamwork mindset and willingness to pitch in where needed
