Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and on schedule
Document clinical observations and chart patient progress clearly
Collaborate with the care team to develop and follow individualized care plans
Monitor patients for changes in condition and escalate concerns promptly
Build trusting relationships with residents and their families through respectful communication

Qualifications

Active LPN license in Pennsylvania (PA) required
Graduate of an accredited Practical Nursing program (LPN)
Current CPR/BLS certification (or willingness to obtain)
Previous experience in long-term care or skilled nursing preferred
Strong communication, teamwork, and patient-centered care skills

Education

Practical Nursing diploma or equivalent (LPN program graduate)

Knowledge & Skills

Medication administration and basic IV/assessment familiarity
Patient documentation and electronic charting best practices
Vital signs monitoring and basic clinical assessments
Infection control and safe patient handling principles
Ability to prioritize tasks in a fast-paced care setting
