Contract Type: Per-Diem

Shift Type: Per diem, 8-hour shifts; schedule varies and will be coordinated with the facility (exact start/end times and days are determined by facility needs)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a long-term care setting
Administer medications and treatments accurately and safely
Assess residents' conditions, record observations, and report changes to the care team
Document care and interventions clearly in the facility charting system
Collaborate with interdisciplinary team members and communicate with residents and families
Support care planning and help implement individualized care interventions
Follow infection control, safety protocols, and facility policies to ensure quality care

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (preferred or required depending on facility)
Prior experience in long-term care or similar setting preferred
Up-to-date immunizations and TB screening per facility policy
Reliable, patient-focused, and able to work flexible per-diem shifts

Education

Practical Nursing diploma or equivalent; LPN certification

Knowledge & Skills

Medication administration and documentation
Basic patient assessment and vital sign monitoring
Wound care basics and assistance with ADLs
Electronic charting and clear clinical documentation
Strong communication, teamwork, and time-management skills
Familiarity with infection control practices and resident safety
