Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents, following care plans and facility policies
- Administer medications and treatments accurately and document all care clearly
- Monitor patient condition and communicate changes promptly to RN/clinical leadership
- Assist with daily living activities while promoting dignity and independence
- Collaborate with interdisciplinary team members to support excellent resident outcomes
- Maintain accurate and timely documentation in resident records
- Participate in quality improvement and infection control practices to keep everyone safe
Qualifications
- Current LPN license in Pennsylvania (required)
- Completion of an accredited practical nursing program
- Basic Life Support (BLS) certification preferred or willingness to obtain
- Strong communication skills and a team-oriented mindset
- Reliable, professional, and committed to resident-centered care
Education
- High school diploma or equivalent
- Graduation from an accredited Practical Nursing (LPN) program
Knowledge & Skills
- Clinical assessment and bedside care for adult and geriatric populations
- Medication administration and documentation best practices
- Familiarity with charting and electronic health record systems
- Infection control principles and safe patient handling
- Good time management and ability to prioritize clinical tasks
- Strong interpersonal skills for working with residents and families
