Roles & Responsibilities

Provide compassionate bedside care and routine clinical assessments to residents
Administer medications and treatments accurately and document appropriately
Collaborate with the care team to develop and follow individualized care plans
Communicate clearly with patients, families, and interdisciplinary staff to promote quality outcomes
Monitor and report changes in patient condition promptly
Support admission, transfer, and discharge processes with professionalism

Qualifications

Current Licensed Practical Nurse (LPN) license in Pennsylvania required
BLS or CPR certification required; additional certifications a plus
Comfortable working 8.5-hour clinical shifts and providing direct patient care
Clear background check and any facility-required health screenings
Strong communication skills and a teamwork mindset; recent clinical experience preferred

Education

LPN diploma or vocational nursing certificate

Knowledge & Skills

Basic nursing assessment and patient monitoring skills
Medication administration and accurate documentation
Familiarity with electronic health record documentation
Ability to follow care plans and physician orders
Good time management, prioritization, and infection control practices
