Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with LPN scope
• Administer medications, document treatments, and monitor resident responses
• Collaborate with nursing staff and interdisciplinary team to support individualized care plans
• Perform assessments, report changes in condition, and escalate concerns promptly
• Maintain accurate, timely charting and follow facility policies and procedures
• Support a safe, clean environment and assist with resident mobility and activities of daily living
• Build trusting relationships with residents and families to enhance quality of life

Qualifications

• Current LPN license, active in good standing (Pennsylvania licensure preferred)
• Minimum clinical experience as an LPN in long-term care or similar setting preferred
• Current BLS/CPR certification or willing to obtain prior to start
• Reliable, professional, and able to work independently on per diem assignments
• Clear background check and any facility-mandated immunizations or screenings

Education

• Graduate of an accredited practical nursing program or equivalent diploma

Knowledge & Skills

• Strong medication administration and documentation skills
• Knowledge of infection control, wound care basics, and fall prevention
• Good communication and teamwork skills with residents, families, and staff
• Ability to prioritize, multitask, and respond calmly in clinical situations
• Comfortable using electronic health records and standard nursing equipment
