Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and safely
Perform routine assessments and report changes in condition to the care team
Document care activities thoroughly and maintain accurate patient records
Collaborate with RNs, CNAs, therapists, and leadership to deliver coordinated care
Support resident comfort, dignity, and quality of life through attentive care

Qualifications

Valid LPN license for Pennsylvania required
Current CPR/BLS certification or willingness to obtain upon hire
Comfortable administering medications and basic nursing treatments
Strong communication skills and a collaborative attitude
Previous experience in long-term care, skilled nursing, or similar setting preferred

Education

Graduate of an accredited Practical Nursing (LPN) program or equivalent

Knowledge & Skills

Medication administration and basic nursing procedures
Patient assessment and monitoring
Clear documentation practices and charting accuracy
Ability to follow care plans and escalate clinical concerns appropriately
Teamwork, time management, and patient-centered focus
