Roles & Responsibilities

• Provide compassionate direct patient care consistent with LPN scope of practice
• Administer medications and treatments accurately and document thoroughly
• Monitor and record vital signs, observe patient condition, and report changes
• Assist with wound care, ADLs and other bedside nursing tasks as needed
• Collaborate with RNs, providers and interdisciplinary team to support care plans
• Maintain clear, timely charting in the electronic medical record and reports
• Follow facility policies, safety protocols and infection control standards
• Support patient and family education with empathy and clear communication

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Basic Life Support (BLS) certification or willingness to obtain prior to start
• Current immunizations and ability to pass background and health screenings
• Excellent clinical judgment, organization and communication skills
• Previous experience in long-term care or similar settings is a plus

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Patient assessment basics and recognizing changes in condition
• Comfortable with charting in electronic medical records
• Strong teamwork, time management and patient-centered communication
• Familiarity with infection control and safety protocols
