Shift Type: Full-time PM shift, 2:00 PM - 10:00 PM; includes every other weekend (evening/PM shift)
Hours per Week: 36
Candidate Pay Rate: $45per hour

Roles & Responsibilities
- Administer medications to residents following the five rights and facility policy
- Assess residents regularly and report any changes in condition to the charge nurse
- Communicate changes and concerns to physicians and update family members as needed
- Assist CNAs with resident care tasks and respond to call lights as a supportive team member
- Document care and reporting accurately and timely to maintain continuity of care
- Report any allegations of abuse, neglect, or misappropriation to leadership promptly
- Perform other clinical duties as requested to support resident wellbeing
Qualifications
- Current Wisconsin LPN license (required)
- Current CPR/BLS certification (required)
- 1+ years of related LPN experience preferred, ideally in long-term care
- Demonstrated accuracy and strong attention to detail
- Comfortable administering medications and performing resident assessments
- Strong communication skills for interacting with residents, families, CNAs, and physicians
Education
- Graduate of an accredited practical nursing program or equivalent; an equivalent combination of education, training, and experience will be considered
Knowledge & Skills
- Medication administration and pass procedures
- Resident assessment and change-of-condition recognition
- Clear, compassionate communication with families and physicians
- Accurate reporting and documentation practices
- Ability to collaborate with CNAs and the interdisciplinary care team
- Basic emergency response and CPR/BLS skills
