Roles & Responsibilities

• Assess patients' speech, language, cognitive-communication, and swallowing needs within the skilled nursing setting
• Develop and implement individualized, evidence-based treatment plans that support functional outcomes
• Deliver skilled therapy services with compassion and clinical precision
• Document evaluations, treatment plans, and progress notes in a timely manner to meet facility and CRG expectations
• Collaborate closely with the interdisciplinary team to coordinate care and discharge planning
• Maintain productivity standards and meet the facility's 90% productivity requirement
• Adhere to CRG policies and facility dress code (black scrubs) while representing the therapy team professionally

Qualifications

• Licensed/credentialed Speech-Language Pathologist (SLP) as required by state and facility
• 1-3 years of SLP experience (experience in Skilled Nursing Facility settings strongly aligned to this role)
• Ability to work on-site and be present to receive guaranteed hours
• Comfortable following established policies and meeting productivity expectations
• Strong communication and documentation skills

Knowledge & Skills

• Skilled in adult/geriatric SLP assessment and intervention
• Experience with dysphagia management and safe swallow techniques
• Knowledge of SNF workflows and interdisciplinary rehabilitation practices
• Efficient documentation and time-management to support productivity goals
• Ability to build rapport with residents and family members
