Roles & Responsibilities
- Provide compassionate direct patient care following facility policies and care plans
- Administer medications and treatments accurately and document thoroughly
- Monitor and report changes in patient condition to the care team promptly
- Collaborate with nurses, providers, and support staff to deliver coordinated care
- Assist with admissions, discharges, and transitions to keep patient flow smooth
- Support patients and families with clear communication and empathetic care
Qualifications
- Active Licensed Practical Nurse (LPN) license in good standing
- Prior LPN experience in a long-term care or skilled nursing setting preferred
- Current BLS/CPR certification preferred or willing to obtain
- Ability to work the posted day shift hours (5:45 AM - 6:15 PM)
- Local candidate or within commuting distance (assignment is LOCAL ONLY)
- Strong communication skills and a team-first attitude
Education
- LPN diploma or equivalent
- High school diploma or GED
Knowledge & Skills
- Clinical assessment and basic nursing interventions
- Medication administration and accurate charting
- Familiarity with long-term care workflows and documentation
- Patient safety practices and infection control
- Time management and prioritization during busy shifts
- Collaborative teamwork and effective communication
