Roles & Responsibilities

Provide compassionate direct patient care following facility policies and care plans
Administer medications and treatments accurately and document thoroughly
Monitor and report changes in patient condition to the care team promptly
Collaborate with nurses, providers, and support staff to deliver coordinated care
Assist with admissions, discharges, and transitions to keep patient flow smooth
Support patients and families with clear communication and empathetic care

Qualifications

Active Licensed Practical Nurse (LPN) license in good standing
Prior LPN experience in a long-term care or skilled nursing setting preferred
Current BLS/CPR certification preferred or willing to obtain
Ability to work the posted day shift hours (5:45 AM - 6:15 PM)
Local candidate or within commuting distance (assignment is LOCAL ONLY)
Strong communication skills and a team-first attitude

Education

LPN diploma or equivalent
High school diploma or GED

Knowledge & Skills

Clinical assessment and basic nursing interventions
Medication administration and accurate charting
Familiarity with long-term care workflows and documentation
Patient safety practices and infection control
Time management and prioritization during busy shifts
Collaborative teamwork and effective communication
