Roles & Responsibilities
- Verify and dispense medications accurately and safely for inpatient and outpatient needs
- Perform thorough medication reviews and drug utilization evaluations to optimize therapy
- Collaborate with physicians, nurses and other clinicians to support patient-centered care
- Manage controlled substance inventory and maintain regulatory compliance
- Counsel patients and clinical staff on proper medication use, side effects and interactions
- Participate in pharmacy quality improvement initiatives and medication safety programs
- Document interventions and maintain accurate patient and pharmacy records
Qualifications
- Current Pharmacist license in state of assignment or eligibility to obtain licensure promptly
- Minimum 2 years of recent pharmacy experience as indicated in the job posting
- Experience in hospital/inpatient pharmacy practice preferred
- Knowledge of sterile and non-sterile compounding practices preferred
- Strong communication skills and the ability to work collaboratively in multidisciplinary teams
- Basic life support or other facility-required certifications as applicable
Education
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited program
Knowledge & Skills
- Medication dispensing, dose verification and IV preparation skills
- Familiarity with pharmacy information systems and electronic medical records
- Medication safety practices, controlled substance handling and regulatory compliance
- Clinical knowledge for medication therapy management and drug interactions
- Attention to detail, time management and ability to prioritize in a fast-paced setting
