Roles & Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans
- Provide skilled therapy focused on mobility, strength, balance, and functional independence
- Document therapy interventions and patient progress accurately and in a timely manner
- Collaborate with interdisciplinary teams to support optimal patient outcomes
- Educate patients and families on therapeutic exercises, safety, and mobility strategies
- Maintain organized, safe, and compliant treatment areas
- Ensure adherence to facility policies and state regulations
Qualifications
- Active California Physical Therapist (PT) license required
- Prior SNF or rehab experience preferred
- Strong communication, clinical reasoning, and documentation skills
- Availability for the full 13-week contract assignment
- Ability to work independently and as part of a collaborative team
Knowledge & Skills
- Skilled in patient evaluation and developing individualized treatment plans
- Expertise in mobility, strength, balance, and functional rehabilitation
- Clear and timely therapy documentation and clinical reasoning
- Comfortable collaborating with interdisciplinary teams
- Able to educate patients and families effectively about safety and exercises
