Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop patient-centered treatment plans
• Deliver compassionate, evidence-based care in a skilled nursing facility setting
• Collaborate with interdisciplinary team members to optimize patient outcomes
• Document clinical progress accurately and communicate changes in condition
• Coach patients and families on mobility, safety, and functional independence
• Participate in care plan meetings and contribute to quality improvement efforts

Qualifications

• PT or DPT (listed as job skills)
• Open to all experience levels — any experience, including new grads, is acceptable
• Comfortable practicing in a Skilled Nursing Facility (SNF) environment
• Able to work a contract travel assignment (13-week contract noted in job details)
