Roles & Responsibilities

• Provide compassionate case management and discharge planning for post-acute patients
• Conduct psychosocial assessments to identify patient and family needs
• Coordinate community resources and referrals to support transitions of care
• Advocate for patients and families to ensure access to necessary services
• Document clinical encounters accurately in Cerner and maintain timely charting
• Collaborate with interdisciplinary teams to develop and implement care plans
• Support crisis intervention and safety planning when urgent needs arise

Qualifications

• MSW (Master of Social Work) required
• LCSW preferred (encouraged but not mandatory)
• BLS (AHA) certification required
• Minimum 2 years of social work experience, ideally in post-acute or hospital settings
• Demonstrated ability to use Cerner or similar electronic health record systems
• Active state social work license and ability to submit (per facility requirements)

Education

• MSW (Master of Social Work)

Knowledge & Skills

• Strong knowledge of discharge planning, community resources, and case management
• Skilled in crisis intervention, mental health screening, and patient advocacy
• Proficient with Cerner charting and accurate clinical documentation
• Excellent communication and interdisciplinary collaboration skills
• Ability to manage caseloads independently and prioritize time-sensitive tasks
• Cultural sensitivity and ability to work with diverse patient populations
