Roles & Responsibilities

• Coordinate patient care across the continuum as a hands-on Care Coordinator and RN
• Perform utilization review and case management to ensure appropriate, timely care
• Document thoroughly and accurately in Cerner to maintain clear patient records
• Collaborate with interdisciplinary teams to develop discharge and care plans
• Triage and prioritize cases, applying analytical and problem-solving skills
• Educate patients and families on care plans, transitions, and community resources
• Support unit operations by floating as needed and adapting to changing priorities

Qualifications

• Active California RN license required
• Minimum of 3 years nursing experience in an acute hospital setting
• Prior care coordination or case management experience in a clinical setting is required
• BLS Certification required
• COVID vaccine required
• Crisis Protocol approved (required)
• Experience with Cerner charting system preferred
• Strong customer service, presentation, interpersonal and written communication skills

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) — BSN preferred but not required
• Active RN license in California

Knowledge & Skills

• Care Coordination and Case Management best practices
• Utilization Review processes and documentation
• Acute care clinical skills and triage
• Proficiency with Cerner electronic health records
• Excellent analytical thinking and problem-solving abilities
• Project/task management and ability to manage multiple priorities
• Ability to work independently and to float between assignments
• Comfort working in a Trauma Level 3 acute care environment
