Shift Type: Day shifts, 10-hour shifts (typically 4x10 to meet 40 hours/week), Every Other Weekend (EOW), First to Float. Exact facility start/end times vary by unit.

Candidate Pay Rate: $82 per hour

Roles & Responsibilities
- Coordinate patient care across the continuum as a hands-on Care Coordinator and RN
- Perform utilization review and case management to ensure appropriate, timely care
- Document thoroughly and accurately in Cerner to maintain clear patient records
- Collaborate with interdisciplinary teams to develop discharge and care plans
- Triage and prioritize cases, applying analytical and problem-solving skills
- Educate patients and families on care plans, transitions, and community resources
- Support unit operations by floating as needed and adapting to changing priorities
Qualifications
- Active California RN license required
- Minimum of 3 years nursing experience in an acute hospital setting
- Prior care coordination or case management experience in a clinical setting is required
- BLS Certification required
- COVID vaccine required
- Crisis Protocol approved (required)
- Experience with Cerner charting system preferred
- Strong customer service, presentation, interpersonal and written communication skills
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) — BSN preferred but not required
- Active RN license in California
Knowledge & Skills
- Care Coordination and Case Management best practices
- Utilization Review processes and documentation
- Acute care clinical skills and triage
- Proficiency with Cerner electronic health records
- Excellent analytical thinking and problem-solving abilities
- Project/task management and ability to manage multiple priorities
- Ability to work independently and to float between assignments
- Comfort working in a Trauma Level 3 acute care environment
