Roles & Responsibilities

• Assist and supervise day-to-day nursing activities during night tours of duty
• Provide high-quality patient care in accordance with federal, state and local regulations
• Support rehabilitative and restorative nursing programs to help residents maintain independence
• Coordinate with the Director of Nursing to ensure regulatory compliance and quality standards
• Supervise and mentor clinical staff on the floor during your shift
• Accurately document resident condition, treatments and interventions in the medical record
• Administer medications and treatments safely; perform IV care if certified
• Communicate clearly with interdisciplinary team members and families to support resident-centered care

Qualifications

• Current LPN license in the state of Colorado (required)
• Current BLS certification (required)
• Minimum 3 years of clinical experience in a hospital, skilled nursing facility, or comparable setting
• IV certification preferred for LPNs
• Experience in long-term care and rehabilitative/restorative nursing
• Night shift experience and the ability to work independently during overnight hours

Education

• Vocational nursing diploma or equivalent training leading to LPN licensure

Knowledge & Skills

• Strong patient care skills with a focus on long-term care and rehabilitation
• Knowledge of restorative nursing practices and rehabilitative care plans
• Familiarity with skilled nursing facility operations and regulatory compliance
• Clinical supervision and mentoring skills
• Medication administration, documentation, and assessment skills
• Effective communication with residents, families, and interdisciplinary teams
