Roles & Responsibilities
- Provide compassionate, competent nursing care to PACE participants in accordance with plan of care
- Administer medications and treatments safely and accurately
- Perform focused assessments and document clinical findings clearly and promptly
- Communicate effectively with the interdisciplinary care team to coordinate participant needs
- Assist with activities of daily living and promote participant independence when possible
- Adhere to infection control procedures and facility policies, including required health screenings
- Support participant-centered care planning and family communication
Qualifications
- Active LPN license in the state of assignment (Indiana license required when working in Indiana)
- Current BLS (Basic Life Support) certification
- Successful completion of license/certificate verification prior to start
- Ability to pass required health screenings and immunizations (COVID-19, Influenza, MMR, Hepatitis B, Varicella, Tdap)
- Clear drug screen and required health documentation (Trinity 10 Panel, physical/health statement, TB testing or questionnaire)
- Fit test clearance (for applicable respirators) and completion of facility-specific health questionnaires
- Strong communication skills and comfort working in an interdisciplinary team
Education
- Graduate of an accredited practical nursing program or equivalent
- Active LPN license required
Knowledge & Skills
- Clinical skills in medication administration and basic nursing procedures
- Knowledge of infection prevention and immunization requirements
- Competence with documentation and electronic health records (facility-specific training provided)
- Ability to perform focused patient assessments and report changes
- Good time management and the flexibility to adapt to per diem scheduling
- Strong interpersonal skills and an emphasis on dignity-centered care for older adults
