Roles & Responsibilities

• Lead utilization review and chart audits to ensure regulatory compliance and high-quality care
• Implement and evaluate performance improvement and quality initiatives across the caseload
• Review medical records including plans of care and progress notes for compliance and best practice
• Monitor OASIS data integrity and complete OASIS data submission accurately and timely
• Facilitate interdisciplinary case conferences to coordinate patient care and ensure plan of care development
• Produce monthly quality and public-reportable data to guide service improvements
• Support evaluation of clinical competence and ensure staff compliance with home health standards
• Promote a clean, safe care environment and uphold patient privacy and confidentiality standards

Qualifications

• Minimum 2 years of Home Health experience (required)
• Prior Quality experience required — experience with utilization review and chart audits
• Active Certificate for OASIS Specialist - Clinical (COS-C) (required)
• Active Illinois nursing license (must include agency's IL nursing license in submission)
• Valid driver’s license and proof of car insurance (required)
• OASIS experience (required)
• CPR certification (required)
• Current COVID status/card (declinations will not be approved)
• Resume, skills checklist (within 1 year), 2 supervisory references, and active certifications/licenses required for submission

Education

• Active Illinois Registered Nurse (RN) license (documentation required at submission)

Knowledge & Skills

• Regulatory compliance and quality reporting in home health settings
• Utilization review and chart audit processes
• OASIS data integrity, submission, and COS-C best practices
• Care coordination and individualized plan of care development
• Performance improvement methodologies and producing quality metrics
• Experience with Matrix EMR (iPad provided) and telehealth/virtual visit workflows
• Wound/ostomy care, infusion management, and chronic disease management exposure
• Supervisory and case management experience helpful for interdisciplinary coordination
