Roles & Responsibilities

• Evaluate patients in the home setting to assess function and create individualized care plans
• Develop and implement medically prescribed physical therapy programs to restore function and limit disability
• Document observations, clinical findings, skilled interventions, goals and discharge plans thoroughly and timely
• Perform admission/evaluation visits and notify the Clinical Team Manager of OASIS scores within established timelines
• Supervise Physical Therapy Assistants as appropriate to ensure high-quality patient care
• Coordinate with the interdisciplinary team to support patient-centered outcomes and smooth transitions of care

Qualifications

• Active Physical Therapist licensure (state licensure required; ability to obtain if relocating)
• Minimum one year of Home Health experience (explicit requirement)
• Familiarity with OASIS documentation and timelines
• Experience supervising Physical Therapy Assistants or comfort mentoring clinical staff
• Strong clinical assessment and documentation skills
• Willingness to travel within the assigned territory (Atherton, Redwood City, Belmont, San Mateo, Burlingame, Millbrae, San Bruno and nearby cities)

Education

• Doctor of Physical Therapy (DPT) or entry-level Physical Therapy degree (or equivalent)
