Shift Type: Monday-Friday, 8:00 AM - 5:30 PM (Flexible based on patient availability); one weekend day per month — Day shift
Hours per Week: 40
Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Evaluate patients in the home setting to assess function and create individualized care plans
- Develop and implement medically prescribed physical therapy programs to restore function and limit disability
- Document observations, clinical findings, skilled interventions, goals and discharge plans thoroughly and timely
- Perform admission/evaluation visits and notify the Clinical Team Manager of OASIS scores within established timelines
- Supervise Physical Therapy Assistants as appropriate to ensure high-quality patient care
- Coordinate with the interdisciplinary team to support patient-centered outcomes and smooth transitions of care
Qualifications
- Active Physical Therapist licensure (state licensure required; ability to obtain if relocating)
- Minimum one year of Home Health experience (explicit requirement)
- Familiarity with OASIS documentation and timelines
- Experience supervising Physical Therapy Assistants or comfort mentoring clinical staff
- Strong clinical assessment and documentation skills
- Willingness to travel within the assigned territory (Atherton, Redwood City, Belmont, San Mateo, Burlingame, Millbrae, San Bruno and nearby cities)
Education
- Doctor of Physical Therapy (DPT) or entry-level Physical Therapy degree (or equivalent)

- Patient evaluation and clinical assessment
- Treatment planning and skilled interventions in home health
- OASIS scoring and timely documentation
- Goal setting and individualized discharge planning
- Admission/evaluation visits and ongoing patient reassessment
