Roles & Responsibilities

• Evaluate patients in the home setting to identify functional limitations and develop individualized treatment plans
• Provide skilled therapeutic interventions to restore or maximize patient function
• Document clinical findings, skilled interventions, goals and discharge plans in a timely, compliant manner
• Perform initial admission and evaluation visits and communicate OASIS scores to the Clinical Team Manager within established timelines
• Supervise Physical Therapy Assistants as needed to ensure high-quality, coordinated patient care
• Collaborate with interdisciplinary home health and hospice teams to support patient-centered outcomes

Qualifications

• One year of home health experience (required)
• Current state Physical Therapist license (required)
• Demonstrated ability to perform initial admission/evaluation visits and OASIS-related documentation
• Experience supervising Physical Therapy Assistants is a plus
• Strong documentation skills and ability to communicate effectively with clinical teams and patients

Education

• Graduation from an accredited Physical Therapy program (DPT preferred)

• Thorough documentation practices including OASIS familiarity
• Discharge planning and care coordination
• Comfort working independently across a defined territory and collaborating with a clinical team
