Roles & Responsibilities
- Evaluate and assess patients to establish baseline function and identify needs
- Develop individualized treatment plans focused on activities of daily living and safety at home
- Deliver skilled occupational therapy interventions to restore and maximize function
- Provide education and training to patients, families, and caregivers to support independence
- Document clinical findings, interventions, and progress in a timely and compliant manner
- Collaborate with interdisciplinary team members to coordinate care and optimize outcomes
- Perform safe travel to patient homes across the assigned Silicon Valley territory
Qualifications
- Current, active California Occupational Therapist license (required)
- OTR (Registered Occupational Therapist) credential preferred
- Experience in home health and hospice settings is highly valued
- Comfortable performing patient evaluations, treatment planning, and ADL training
- Valid driver’s license and reliable transportation to cover the assigned territory
- Strong clinical judgment, documentation skills, and ability to work independently
Education
- Bachelor’s or Master’s degree in Occupational Therapy (entry-level OT degree required; OTD a plus)
Knowledge & Skills
- Skilled in Activities of Daily Living (ADL) assessment and treatment
- Proficient in patient evaluation and individualized treatment planning
- Knowledge of home safety assessments and adaptive equipment recommendations
- Familiarity with hospice care principles and end-of-life comfort-focused interventions
- Excellent communication, patient education, and interdisciplinary collaboration skills
- Time management and clinical documentation proficiency
