Roles & Responsibilities
- Evaluate residents and establish individualized therapeutic plans of care with clear long- and short-term goals
- Deliver hands-on and modality-based treatments that maximize residents’ functional independence
- Document all evaluative and therapeutic interactions clearly and promptly in the EMR per department protocols
- Recommend, fit, and educate residents on assistive devices to support safe mobility and ADLs
- Collaborate with physicians, nursing, and the interdisciplinary team to coordinate care and discharge planning
- Monitor treatment effectiveness and progress plans of care, modifying interventions as resident needs change
- Supervise and mentor assistant-level staff and support their development during orientation and beyond
- Provide in-service education and contribute to development of new rehabilitation programs and services
Qualifications
- Active Physical Therapist license in the state of Rhode Island (required)
- Bachelor’s degree required; Master’s or Doctorate in Physical Therapy preferred
- Basic Life Support (BLS) certification preferred
- Experience in long-term care or skilled nursing settings preferred
- Strong documentation skills with EMR experience and knowledge of Medicare/Medicaid documentation requirements
- Ability to lift and assist residents (able to lift up to 50 pounds; two-person lift for heavier transfers)
- Comfort supervising assistant-level staff and working within state/professional supervision guidelines
- Excellent communication and interpersonal skills to engage residents and families effectively
Education
- Bachelor’s degree required (PT)
- Master’s or Doctorate in Physical Therapy preferred
Knowledge & Skills
- Resident-centered care approach with strong clinical reasoning and analytical skills
- Proficient in Microsoft Office and electronic medical record documentation
- Solid understanding of Medicare, Medicaid, and private pay documentation and compliance
- Strong time management, organization, and presentation skills
- Ability to work collaboratively within an interdisciplinary team and provide clear consultation
- Compassionate bedside manner and the ability to teach residents and caregivers effectively
- Fluent in English for reading, writing, and oral instruction as required by the role
