Roles & Responsibilities
- Provide comprehensive patient evaluations and create individualized OT treatment plans
- Perform revisits and ongoing skilled interventions to meet patient goals
- Complete timely and accurate documentation in Cerner after each visit
- Manage an average caseload of four visits per day combining evaluations and revisits
- Coordinate care with supervisors, nursing staff, and other interdisciplinary team members
- Track and submit mileage timesheets per GSA guidelines and follow local approval procedures
- Maintain professional communication with patients and families during home visits
- Participate in required meetings and sign-off processes to ensure billing and compliance
Qualifications
- Current California Occupational Therapy license (required)
- BLS certification (required)
- Active NPI number (required)
- Valid driver’s license and proof of automobile insurance (required for home health)
- Minimum 2 years of experience in a healthcare setting
- Cerner charting experience preferred
- Two supervisory references required at submittal (one within 12 months and one within 3 years)
- COTA with strong home health experience will be considered if OT applicants are unavailable
Education
- Graduation from an accredited Occupational Therapy program (required for licensure)
Knowledge & Skills
- Strong clinical assessment and treatment planning skills in a home health setting
- Comfortable performing evaluations and skilled revisits in patient homes
- Proficient with Cerner or similar electronic health record systems
- Knowledge of GSA mileage billing and accurate timesheet practices
- Excellent time management and ability to handle a mobile caseload
- Clear documentation and communication skills with patients and supervisors
- Ability to navigate the coverage area (NE Sacramento into Folsom) safely and efficiently
