Roles & Responsibilities

• Provide personalized, high-quality rehabilitative services and evaluations for the outpatient caseload
• Document patient progress and outcomes accurately using Net Health and facility systems
• Build meaningful, long-term relationships with residents and empower them to stay engaged in their wellness
• Educate patients, families, and staff about therapy programs, goals, and expected outcomes
• Collaborate with fellow therapists and facility team members to deliver coordinated care
• Maintain a professional, compassionate, and patient-centered approach at all times

Qualifications

• Degree in Physical Therapy from an accredited program
• Active Physical Therapist license in Pennsylvania in good standing
• Genuine passion for working with the senior population
• Strong communication and collaboration skills
• Team-oriented mindset with a positive, professional demeanor
• W2 or C2C employment ability as indicated (per posting)

Education

• Degree in Physical Therapy from an accredited program (e.g., DPT preferred)

Knowledge & Skills

• Skilled in patient evaluation and development of individualized treatment plans
• Experience or comfort documenting in Net Health and common facility systems
• Knowledge of rehabilitative services for older adults and outpatient therapy best practices
• Excellent interpersonal skills for resident and family education
• Strong organization and time-management in a busy outpatient setting
