Roles & Responsibilities
- Conduct thorough psychosocial assessments to inform patient-centered care plans
- Coordinate discharge planning and resources to ensure safe transitions home
- Collaborate with nurses, physicians, therapists and the care team to support patients
- Manage a typical caseload of ~15–20 patients, prioritizing needs and follow-up
- Document interventions and care plans in Cerner EMR accurately and timely
- Advocate for patients and families to access community resources and supports
- Participate in general hospital orientation and department-specific onboarding
Qualifications
- MSW (Master of Social Work) required
- Active Washington state social work license and registration with WA DOH (must have)
- Experience with care coordination, discharge planning, and inpatient workflows
- Comfortable using Cerner EMR for documentation
- Ability to work collaboratively within a multidisciplinary team
- No behavioral health assessments or coordination required for this role
Education
- MSW (Master of Social Work)
Knowledge & Skills
- Strong knowledge of psychosocial assessment techniques
- Expertise in discharge planning and community resource linkage
- Familiarity with inpatient hospital operations and workflows (including OB coverage potential)
- Effective communication and documentation skills
- Ability to prioritize a caseload of 15–20 patients and manage time efficiently
